The scrotum in some men extends up the underside of the penis, creating an indistinct junction between the penis and scrotum. This penoscrotal webbing makes the penis appear short on its undersurface. It can also cause discomfort with intercourse or difficulty using a condom. The penoscrotal web may be congenital or due to removal of too much skin from a circumcision, which is the most common cause.
A mild web is eliminated by rearranging the tissues at the penoscrotal junction, which leaves a zigzag incision (Z-plasty). A more severe web is treated by excising the web and making a zigzag closure at the junction of the penis and scrotum. Excision of the web by leaving a linear scar may result in worsening or recurrence of the web due to the scar contracting. Therefore, the scar is broken up to prevent the contraction. This procedure is relatively simple and can be performed under general anesthesia or sedation.
